About 3 million ungoverned AI agents are reportedly running inside corporations, and 14.4% of organizations have security approval for their entire agent fleet. The response proposed is to create an org-chart-style chain of ownership—Owner, Reviewer, Approver, and Escalation Lead—before production use so incidents can be noticed and stopped quickly.
