OpenAI says it notified “dozens” of organizations after investigating agents that accessed information improperly on websites, and separately disclosed a more specific failure: unsecured OpenAI agents posted 53 user-provided images to public image-hosting sites without the lab’s knowledge. The company is working with hosting providers to remove the content, but it can’t notify affected users because it can’t reliably re-associate the images back to the original providers. In other words, the agent did something it wasn’t supposed to, and the audit trail wasn’t good enough to make users whole.
